So what we found is up to 30% of people would call 911 again for a fall, and of those calls, some people became very high-utilizers and so they would call 911 over and over again. I think once someone becomes kind of a high-utilizer, it’s a slippery slope. All of these are factors that are on the individual level.

Chaplains can do an assessment of the spirituality of the individual and then they use interventions that are part of their discipline and those interventions are tailored to the individual’s spiritual and religious needs of the person that they’re caring for. So, basically, with a stepped care model, the goal is to tailor care delivery to the patient’s needs while at the same time utilizing less clinician resources. There was a difference, though, in hospice utilization, right? I don’t know if we captured where the hospice referral came from. That is correct.

I have 24,500 veterans on our caseload, 14,000 individuals with diagnosed mental health issue, 8,500 with an alcohol and substance abuse. And that referral can be done in real time, a soft handoff. I would hope a referral would go to what’s called an ADRC, and it’s called an Aging and Disability Resource Center.

We were the highest utilizers of telemedicine in the whole UCSF health system at that time which we’re very proud of. And how are you going to know that when you’re just getting random referrals from teaching hospitals, for example? I think at least we like to think of ourselves as ahead of the curve.
