Continuity and Relationships Among Patients, Physicians, and Care Teams (CARE PACT) [Secondary data analysis]

Context: A patient’s relationship with their primary care physician (PCP) or advanced practice provider (APP) is instrumental in their healthcare journey. Full results to be presented at the meeting. We also collected organizational characteristics from 19 FQHCs via survey. and the aggregate patient UPC was 0.74.

I think at the age of 91, a patient should have a say, really, in terms of, hey, I can present you with some of the reasons I think we should make a switch, but what do you think? And, you know, this is back and forth between the PCP and the cardiologist. But another layer I would add to it is also maybe more patient centric.
