Developing an intervention to assist older patients communication with their GP practice after discharge from hospital [Community based participatory research]

Objective The GP-MATE study has built a new intervention for family medicine and patients/carers through co-design. Our intervention aims to assist older patients/their carers to have better communication with their family medicine provider about their care after discharge, thereby improving patient safety outcomes.

Role of General Practitioners in Acute Case Handling

Analysis of 500+ urgent GP visits in Germany shows most cases were handled in-office, often driven by fear, distress, or a need for reassurance. Students also recorded patients’ concerns and expectations, which were analyzed using qualitative content analysis. The results showed that 523 cases were collected from 53 GP practices.
