They assessed OAs aged ≥75 years who were screened for frailty and referred by general practitioners (GP) to 1 of 3 Community Specialist Teams for Older Persons (CST-OP) hubs in the Mid-West of Ireland. At 30 days, mortality was 1.0%, Emergency Department (ED) visits were 6.9%, hospitalization was 6.6%, and NH admission was 4.0%.

So we measure things like hospitalizations and ed visits and the like, and those data are a bit weaker, but they’re also, you know, I think they need to be contextualized in what, why we would even want to reduce hospitalizations right. Why would we want to? I mean, let’s focus for a minute on multi morbidity.
