They’re limited because, on the individual level, when you get down to it, the complexities of these stories and the caring involved may lead to these complex relationships, understanding what the goals are, as you had done with your mom.

The idea was to create a robust community-based infrastructure that could help older adults succeed in their homes and communities, whether they were healthy, how to keep them healthy or they were at imminent risk of emergency room visits, hospitalizations, or nursing home placements. I knew them, I developed a relationship with them.
