I think sometimes we don’t necessarily realize the importance in the role that each of us individually as physicians can have with patients both within the exam room but also globally. The patient reached out to me and asked what I thought or recommended because I had built that relationship with that patient, I had built that trust.

That’s where it really gets to this individual risks and balances, individual situation. Because how that is approached can have lasting effects on relationships, unfortunately. So just starting to unpack what’s happening in an individual’s life can maybe then lead to more conversations and assessment.

Chaplains can do an assessment of the spirituality of the individual and then they use interventions that are part of their discipline and those interventions are tailored to the individual’s spiritual and religious needs of the person that they’re caring for. What were those spiritual domains? Is that your grandchildren?

And now when I think about the clinic at Duke, when we’re really running on all cylinders, we can get ourselves up to a five, six, seven week wait for a new palliative care referral. It’s about establishing a relationship and rapport. I don’t know if we captured where the hospice referral came from.

I have 24,500 veterans on our caseload, 14,000 individuals with diagnosed mental health issue, 8,500 with an alcohol and substance abuse. And that referral can be done in real time, a soft handoff. I would hope a referral would go to what’s called an ADRC, and it’s called an Aging and Disability Resource Center.

Ideally the conversation would happen before, but also clinicians were able to make referrals to facilitators.
