A referral order was created in the hospital system’s electronic health record to the "Hospital Discharge Clinic," and referrals were placed for urgent (7-day) or routine (14-day) TCM appointments.

The insight started when Carmen, an orthopedic surgeon-researcher, and Katie, a physical therapist- researcher participated in ride-alongs with EMS providers to patient’s homes. And I think one key part is educating the field, not just my fellow cardiologists, but also primary care doctors, geriatricians, that there’s this heart failure syndrome that is kind of sneaky, and it’s kind of difficult to diagnose any. And, you know, this is back and forth between the PCP and the cardiologist.
