I had signed onto his chart hoping he would be a quick in-and-out patient among the endless sea of critically-ill patients that arrived at our doors that evening. Over my numerous years as an ER doctor, I have met people that I never would have had the fortune, or misfortune (as some would say), to otherwise meet.

I asked emergency medicine clinicians what they thought when a patient who is seriously ill and DNR comes to the ED, and some responded, (paraphrasing), what are they doing here? Most emergency providers wanted to do the right thing for seriously ill patients, but they didnt have the knowledge, skills, or experience to do it.

So if you think about 100 patients per year, that you need to have some benchmark after going into business to care for so many terminally ill patients. I came to work for VITAS because I needed to move to Florida because my dad had a serious illness, metastatic colon cancer. What I love about VITAS is I report to the CEO.

You know, she would have had 90 really good years, and she would have just gone into a coma with no blood pressure and died, you know, with, like, a day and a half of illness. So elderly people who aspirated, got pneumonia, had an mi, didn’t get hauled off to the emergency room on an ambulance crew so they could die in the ER.
