I asked emergency medicine clinicians what they thought when a patient who is seriously ill and DNR comes to the ED, and some responded, (paraphrasing), what are they doing here? This is not why I went into emergency medicine. I went into emergency medicine to act. Why is that important in the emergency room?

So elderly people who aspirated, got pneumonia, had an mi, didn’t get hauled off to the emergency room on an ambulance crew so they could die in the ER. And so I thought about it a while, called, and sure enough, they brought him up to the emergency room. That’s why we did it. And I had his address.

And we would provide ongoing longitudinal care to them in the home, much like the Mount Sinai Visiting Doctors programs, and other programs like that. And the treatments are different than what hospice might provide in the home setting. And then, the patient can bypass the emergency room entirely. I trained at Hopkins.

Including, I think, a 24/7 palliative care intervention in the emergency room, an inpatient comfort care intervention for COVID-positive patients … doing a tremendous amount. So we anticipated, based on what was happening in Seattle, a pretty large influx of elders that were going to come through the emergency room.
