Context: The Transitional Care Management (TCM) clinic visit is a uniquely billed visit type to review a recently discharged patient’s hospital course, reconcile medications, and continue ongoing workup. Objective: Our objective was to improve the TCM clinic no-show rate and thereby improve patient outcomes.

Intervention/Instrument: We collected basic eConsult service utilization data, including the specialty group consulted and specialist response time, as well as the PCP’s responses to a close-out survey to describe their experience with eConsult. Specialists responded to all cases in a median of less than 1.2
