Context: The Transitional Care Management (TCM) clinic visit is a uniquely billed visit type to review a recently discharged patient’s hospital course, reconcile medications, and continue ongoing workup. Population Studied: Inpatients were referred if they were being discharged from the Family Medicine service to home.

Objective: To determine how primary care providers (PCP) utilize the eConsult platform. Setting or Dataset: CUSOM and University Hospital. Population Studied: CUSOM PCPs. Outcome Measures: The number of eConsults by PCP and type of eConsult sent. 1,087 unique PCPs ordered at least 1 eConsult across 28 specialties.

Live discharges may be problematic if they lead to high rates of hospice readmissions, high rates of hospital utilization following discharge, or occur after 180 days in hospice. Outcome Measures: Emergency department utilization, hospitalization, steroid prescriptions, and primary care provider (PCP) utilization. 6-month pre/post evaluation, although not statistically significant, showed ED utilization, hospitalization, and steroid prescriptions decreased 30% to 25% ([95%]CI -0.18 to 0.24, p=0.28).

In Ontario, MD follow-up after ED discharge for chest pain is mostly provided by primary care physicians (PCPs) and sometimes cardiologists.
