Should eligibility and access be determined by clinician referral? By diagnosis? If we move away from clinician referrals as the means by which people get access, how do we keep the clinicians engaged, and not enraged? Should we just use the referrals, whatever’s coming into us, whoever, whatever clinician decides.

Often when there’s a cognitive concern, it’s a family member that is calling saying, “I just want to make sure my mom should still be driving after this happened, or after this diagnosis.” Certainly my bias is that healthcare professionals really do have a role in this discussion. It’s that first place that you start where it may not be the AAAs job, but we’ve organized where they have to have the partnerships and they know who the players are and they know who to make a referral to. And that referral can be done in real time, a soft handoff. And then, the system responds in turn.

Within eight weeks of diagnosis of advanced disease. I don’t know if we captured where the hospice referral came from. Stupid theory.

At a high level to sort of step in and start, you know, tinkering with workflows that, you know, are kind of as healthcare is a giant machine in primary care. Ideally the conversation would happen before, but also clinicians were able to make referrals to facilitators. What’s in the electronic health record? Is that right?

And I’m worried about all of us as healthcare workers, but particularly… And when I say particularly those in the hospital, because they are just tired. If they had a healthcare agent, then this would be better, right? That this is iterative from diagnosis to end-of-life, right? Get the hospice referral.
