It could be in housing, it could be a standalone senior center, home delivered meals, nutrition counseling and nutrition education done by registered dieticians. This includes transportation, driver’s license, parks, volunteerism, housing, and of course health and human services. And then, the system responds in turn. Eric: Okay.

This model will give participating programs a per-member-per-month payment to offer care management, care coordination, and other services such as caregiver training, disease education, and respite. I had the tenacity and education to try to figure this out. They can be nurse if you want to. I had a supportive spouse.

The other way to think about who’s eligible… I talked about coming at this from a diagnosis point of view, people with certain conditions that you need to be hospitalized for. For the most part, programs are doing, twice a day, nursing visits. We’re not paid, as part of a DRG, for any particular diagnosis.
