If we move away from clinician referrals as the means by which people get access, how do we keep the clinicians engaged, and not enraged? We can’t possibly meet the needs of all people with newly diagnosed serious illness. By diagnosis? By prognosis?

So, basically, with a stepped care model, the goal is to tailor care delivery to the patient’s needs while at the same time utilizing less clinician resources.
