When I’m on nursing home call, the most common page I receive is for a blood sugar value. When I’m on palliative care consults and attending in our hospice unit we have to counsel patients about deprescribing and de-intensifying diabetes medications. Eric: Maybe everybody who gets admitted in a nursing home, just boom, CGM.

And so they’re in there for a very short time, couple weeks, get a little bit stronger and transition home after a hospitalization. And so we do have consultants who come in the building. And usually it’s simply for a consultation. Do they need a palliative care doctor, nurse practitioner?

I remember as a fellow, I would come in and our nurse practitioner on our hospice team, I would say the word narcotics, and f or half an hour, she would just lay into me. I see this in the hospital when I’m doing addiction medicine consults. How are we gonna do that if he leaves the hospital?

We might evaluate that in a small number of nursing homes and go from there. They’ve brought on experts, they’ve brought on additional people who are researchers or consultants who do this work all day long, and have added a lot of content.
