Should eligibility and access be determined by clinician referral? If we move away from clinician referrals as the means by which people get access, how do we keep the clinicians engaged, and not enraged? I have never myself called a palliative care consult as a trainee because we didn’t have one. We need more consults.

And now when I think about the clinic at Duke, when we’re really running on all cylinders, we can get ourselves up to a five, six, seven week wait for a new palliative care referral. If they get palliative care consult, they get one. We did not just rely on a physical symptom measure, which many of our colleagues are doing.

And so I’m not sure if this is true for Joe or for Brooke, but even before the pandemic in our practice here, we saw somewhere around 40 to 50% of our patients via telemedicine anyway, because we have a catchment area of patients who live very far away, many of whom have a lot of physical disability and other limitations to coming in person.
