So we measure things like hospitalizations and ed visits and the like, and those data are a bit weaker, but they’re also, you know, I think they need to be contextualized in what, why we would even want to reduce hospitalizations right. We talk about diagnosis, et cetera, but it’s almost more of a community conversation.

I was very close with my grandparents growing up, and then I actually was exposed to geriatrics as a pre-med student at Yale New Haven Hospital as one of the volunteers and Dr. Sharon Inouye’s initial help trial and had a fabulous role model Dr. Verano, who I wanted to be. How do we train primary care doctors to do geriatrics?

And then from that, through community activists, ACT UP, legislators who felt that there needed to be something done, doctors within CDCR who cared passionately about the HIV population and others, all came together and we were able to eventually get funding for this 17-bed inpatient hospice. I’ve worked in hospital settings as an occupational therapist, inpatient rehab and outpatient rehab. Emmy: I was seeing people in the hospital setting, saying they had had a massive brain injury. Emmy: Ended up starting a program, outpatient at our hospital at the time. How did you get interested in this? Emmy: About 25%.

I worry, as a ex-primary care doctor, how much time I have. Rebecca: I think I’ll just say, with the POLST, just like advance care planning, and Susan correct me if I’m wrong here, but I think the POLST was meant to be updated every point of care. It’s much broader than palliative care.
