Most emergency providers wanted to do the right thing for seriously ill patients, but they didnt have the knowledge, skills, or experience to do it. So Tammie developed something called EPIC for Emergency medicine, which is a curriculum that was one of the first ways that I think palliative care got spread in our community.

Many were community hospitals. At one end of the spectrum you can provide decision makers with information that doesn’t work very well in most contexts. We didn’t make them communicate it, but we focused them on it, which makes them more likely to then utilize that information.

I think that policy in the emergency department is showing that no matter what that provider is looking to see if a patient has a preference. If it was going to be for 48 hours to get them through a community acquired pneumonia. What I’d really like to see is that when POLST is utilized, it’s utilized correctly.

Alex: We’ve got a lot to talk about…what states are doing and cities are doing to support older adults in the community. It was designed to really balance what Medicaid at the time was to provide nursing homes and Medicare is obviously health insurance. But, what you need is a community-based game there.

So, basically, with a stepped care model, the goal is to tailor care delivery to the patient’s needs while at the same time utilizing less clinician resources. So at our cancer center, we have physicians, advanced practice providers. And the entire community of palliative care said yes, not a palliative care study.

Don’t get me wrong, the evidence points to cost savings, but as Chris Callahan and Kathleen Unroe pointed out in a JAGS editorial in 2020 “in comprehensive dementia care models, savings may accrue to Medicare, but the expenses accrue to a fluid and unstable network of local service providers, patients, and their families.”

What was life for you as a provider pre-pandemic? We were the highest utilizers of telemedicine in the whole UCSF health system at that time which we’re very proud of.
