So Carmen and Katie developed an EMS Community Partnership program. They created a seamless link between this Community Partnership program and 911 calls for falls. Eric and I enjoyed talking with Carmen and Katie about this innovative and common sense approach to addressing falls in the community. Word of mouth spread rapidly.

Patient just got discharged from the hospital, just coming back from the skilled nursing facility. If you see someone, you know, they have no transportation, they’re single, they have food insecurity, those are important in themselves. And ideally at other times too. And so they’re in there for a very short time, couple weeks, get a little bit stronger and transition home after a hospitalization. The only time that Medicare is paying for nursing homes is in that post acute hospitalization rehab context. So it’s both a facility and a patient incentive to transition from the hospital.

Alex: We’ve got a lot to talk about…what states are doing and cities are doing to support older adults in the community. But, what you need is a community-based game there. Equally as important, is if you wind up in an ED or being hospitalized or wind up in rehab, eventually you’re going to be discharged back home.
