Context In the post-discharge period from hospital acute inpatient care, the older patient is at risk from a lack of joined-up care. Hospitals write discharge summaries to family medicine providers. Objective The GP-MATE study has built a new intervention for family medicine and patients/carers through co-design.

Most individuals retained functional independence for up to 6 months following a community-based CGA. At 30 days, mortality was 1.0%, Emergency Department (ED) visits were 6.9%, hospitalization was 6.6%, and NH admission was 4.0%. Follow-up assessments were completed by an independent assessor via telephone in 30 and 180 days.

So we measure things like hospitalizations and ed visits and the like, and those data are a bit weaker, but they’re also, you know, I think they need to be contextualized in what, why we would even want to reduce hospitalizations right. We talk about diagnosis, et cetera, but it’s almost more of a community conversation.
