So we take that science and we apply it to something that’s very messy and chaotic, like a human being or a community or a population. I think it started off from a need, a hunger in the community. There’s obviously a lot of flaws in the healthcare system. A love letter kind of back to my community.

For example, putting the healthy food at the back of the grocery store is sludge; making an applicant for health insurance climb the flight of stairs to the office – weeding out those less fit – is also sludge. Many were community hospitals. Their family members want them to get out of the hospital sooner.

Well, the irony is that this diethye resurrected from the poorest communities in the mediterranean region. So I lived with a family there. But if your patient is not able to do those things, I wouldn’t say don’t have insurer. The people really farm to table stuff. Lots of fish, lots of vegetables.

Don’t get me wrong, the evidence points to cost savings, but as Chris Callahan and Kathleen Unroe pointed out in a JAGS editorial in 2020 “in comprehensive dementia care models, savings may accrue to Medicare, but the expenses accrue to a fluid and unstable network of local service providers, patients, and their families.” Malaz: I love it.

And so clearly it matters how you bring up the topic, who brings up the topic, and probably the timing when this comes up for patients and families, just like we think about conversations about illness and prognostic understanding, I wish there was a good prevalent state data on how big this is of a problem in serious illness at large.
