Travel to see consultants can be disorienting and difficult for persons living with dementia (PLWD). eConsult is a secure web-based platform that may make communication with specialists more accessible for primary care providers (PCPs). But estimates in community dwelling older adults are around 15%. So, if it’s a hospitalized patient, usually the referral will come through to the SLP service. So, the consult will come in and then we’ll do what we call a clinical bedside evaluation, where we’ll go and see the patient at the bedside.

So, basically, with a stepped care model, the goal is to tailor care delivery to the patient’s needs while at the same time utilizing less clinician resources. If they get palliative care consult, they get one. And the entire community of palliative care said yes, not a palliative care study. It’s not case finding.

We were the highest utilizers of telemedicine in the whole UCSF health system at that time which we’re very proud of. And I think that it was percolating in a lot of the palliative care community. And how are you going to know that when you’re just getting random referrals from teaching hospitals, for example?
