We walked down to the emergency room and reviewed his chart. Then things started to get a little bit different… His chest x-ray had some peripheral infiltrates, and the emergency department providers ordered a CT scan. But, this requires an adequate understanding of the syndrome and having well-defined illness scripts.

Ten years ago we would have been hard pressed to find 3 clinical trial abstracts submitted to the annual meeting, much less high quality randomized trials with robust measures, sample sizes, and analytics plans. Alex: Recently you talked about the other trial, also with a nudge, and that was non-cancer, serious illness. Tom: Thank you.

Can you come and be in our clinic once a week? It used to also be a social worker and myself who would actually go to clinic and spend some time up there. And so I was driven by the fact that I needed more information, and so it sort of became my clinical focus as well as I dabble in the research.

That put me in the emergency room. They just kept at it.

I asked emergency medicine clinicians what they thought when a patient who is seriously ill and DNR comes to the ED, and some responded, (paraphrasing), what are they doing here? This is not why I went into emergency medicine. I went into emergency medicine to act. Why is that important in the emergency room?
