Context: Adults with multiple chronic diseases, mental health comorbidities, and/or social vulnerabilities, may have complex health and social care needs (hereafter "complex needs"). Research is needed to identify mechanisms supporting successful scale-up of integrated care programs for this population.

All of them offer the same array of services, really emphasizing social determinants of health and preventive services like falls prevention, chronic disease, self-management, caregiver training. We have 350,000 grandparents taking care of grandkids. And then, we have nonprofits that are community-based organizations.
