Context: Pre-consultation telemedicine tools have been proposed as an effective solution to optimize the care of the growing number of older patients with chronic conditions. Study design: A qualitative descriptive study design was utilized. Results: We identified 5 barriers/facilitators related to the implementation of ESOGER.

We utilized Amazon Sagemaker Canvas to design our machine learning model. Model design Variables included: blood pressure values, medications, chronic conditions and history of fractures. Historically, most fall risk mitigation efforts occur after the first fall. Evidence-based interventions have been aimed at this population.
