This model will give participating programs a per-member-per-month payment to offer care management, care coordination, and other services such as caregiver training, disease education, and respite. I had the tenacity and education to try to figure this out. They can be nurse if you want to. I had a supportive spouse.

In that clinical experience, we saw a lot of older adults who would develop acute illness, often exacerbations of their chronic conditions. And then, the patient can bypass the emergency room entirely. At our program, we actually require that folks come into the emergency room and have their initial assessment.
