The Minnesota Academy of Family Physicians has been a longtime supporter of HCH, as some of our leaders were involved as the initiative came together. Many of our family medicine physicians work in certified health care homes; they value the infrastructure that the HCH model provides to ensure comprehensive and coordinated care.

We’ve also invited Mike Harper, the Chair of the Board of Directors of the American Geriatrics Society (AGS) as well as past-president for the society (and my previous program director). The fact that we have I think something like 3000 health systems that are at least now somewhat certified and moving to a higher levels of certification.

I am not a board certified. I am not a certified music therapist. And we got all into his family stuff and how he’d been suffering with cancer and his goal. Just encourage patients and families to bring music into their healthcare experience. I let my boards lapse a few years ago. This guy came alive.

Kathleen 03:47 I think any of us who’ve spent any time in nursing homes have spent time working in a nursing home setting, have visited, had family members in nursing homes and looked around and seen the population there. So we see a lot of problems with symptom management, a lot of family conflict. That’s a big question.
