Context: While hypertension is the most common chronic disease cared for during adult primary care visits, it is challenging to know which hypertensive patients have the highest risk of cardiovascular complications in the future. The model was trained on 80% of the data and validated on 20% of the data.

These are patients who had a pre-existing chronic disease, very broadly defined. COPD, heart failure, solid oncology, hematologic malignancy, dementia, ALS, interstitial lung disease, several others. We put a couple extra things in place to really try to reschedule appointments, and this was telehealth. Kate: Sure.
