Context: While hypertension is the most common chronic disease cared for during adult primary care visits, it is challenging to know which hypertensive patients have the highest risk of cardiovascular complications in the future. of those patients have 1-3 primary care visits per year.

These are patients who had a pre-existing chronic disease, very broadly defined. COPD, heart failure, solid oncology, hematologic malignancy, dementia, ALS, interstitial lung disease, several others. People were not as interested in outpatient palliative care. We don’t have enough outpatient palliative care.
