Transitional Care Management care team impact on no-show rates to hospital discharge appointments [Patient education/adherence]

Annals of Family Medicine

Referrals were not placed for patients being discharged to another facility or who already had an outside primary care provider. In the three months (January 2024-March 2024) after implementation, the no-show rates to TCM appointments were 15%, 12%, and 15% respectively, a statistically significant decrease (p-value 0.0011).

Outcomes of a virtual CGM initiation service (virCIS) for primary care patients with diabetes [Diabetes and endocrine disease]

Annals of Family Medicine

Intervention: Primary care providers were invited to attend a webinar that focused on identifying patients who may benefit from CGM use, best practices for prescribing, and CGM interpretation. Patients were provided a CGM, received education on medication, diet and exercise, and completed surveys at baseline, 3, 6, and 12 months.

Rural Ontario Complete Lifestyle Medicine Intervention Program (CLIP-ON) [Dissemination and implementation research]

Annals of Family Medicine

Instrument: Participant and provider progress, satisfaction and feedback were elicited through questionnaires and focus groups. Results: Cohort one of eight participants completed CLIP-ON in April 2024. Conclusion: CLIP-ON has demonstrated patient and provider interest, and logistical feasibility in this rural setting.

Assessment of Project ECHO(R) Opioid Use Disorder Sessions for Primary Care Teams [Education and training]

Intervention: 12 monthly online OUD sessions held from April 2023 to March 2024. Most participants indicated that they would benefit from additional ECHO OUD sessions and that the sessions were valuable to PC providers. Setting: 20 PC practices located in various regions across the US.
