Transitional Care Management care team impact on no-show rates to hospital discharge appointments [Patient education/adherence]

Context: The Transitional Care Management (TCM) clinic visit is a uniquely billed visit type to review a recently discharged patient’s hospital course, reconcile medications, and continue ongoing workup. Referrals were not placed for patients being discharged to another facility or who already had an outside primary care provider.

Outcomes of a virtual CGM initiation service (virCIS) for primary care patients with diabetes [Diabetes and endocrine disease]

Annals of Family Medicine

Intervention: Primary care providers were invited to attend a webinar that focused on identifying patients who may benefit from CGM use, best practices for prescribing, and CGM interpretation. Patients were provided a CGM, received education on medication, diet and exercise, and completed surveys at baseline, 3, 6, and 12 months.
