Referrals were not placed for patients being discharged to another facility or who already had an outside primary care provider.

And interesting enough, this is a really fascinating study that just got published in JAMA I M in December of 2024. Ideally the conversation would happen before, but also clinicians were able to make referrals to facilitators. Sydney 21:41 So we did find increased documentation of the primary outcome in the electronic health records.
