Transitional Care Management care team impact on no-show rates to hospital discharge appointments [Patient education/adherence]

Referrals were not placed for patients being discharged to another facility or who already had an outside primary care provider. In the three months (January 2024-March 2024) after implementation, the no-show rates to TCM appointments were 15%, 12%, and 15% respectively, a statistically significant decrease (p-value 0.0011).

Outcomes of a virtual CGM initiation service (virCIS) for primary care patients with diabetes [Diabetes and endocrine disease]

Intervention: Primary care providers were invited to attend a webinar that focused on identifying patients who may benefit from CGM use, best practices for prescribing, and CGM interpretation. Patients were provided a CGM, received education on medication, diet and exercise, and completed surveys at baseline, 3, 6, and 12 months.
