I wrote this in 2016, when my visits were shorter because that EMR was simpler. But the math is the same, no matter how long your normal appointments are. I have always tried to “squeeze” urgent visits in when I know the patient and the issue they’re having. We also sometimes double book them.

What we did was ask clinicians earlier in the ICU stay for very sick patients to document prognosis, and for those who they thought would survive, to document six-month functional prognosis.
